Applicants must be U.S. citizens or a legal permanent residents; must be members of one of the racial/ethnic minority group (African-American or Black, Asian, Native Hawaiian or other Pacific Islander, American Indian/Alaska Native and Hispanic/Latino); must be enrolled in the final academic year of an accredited or developing professional physical therapist education program; must show evidence of contributions in the areas of minority affairs and services and high scholastic achievement; and must possess potential for superior achievements in the profession of physical therapy as well as professional excellence as a physical therapist. |

Applicants must submit a completed application form along with the following: a copy of an official transcript from the Registrar's Office of all physical therapy coursework (when possible); a personal essay on professional goals and minority service including a response to the three questions listed on the first page of the application; a personal information fact sheet (curriculum vitae); and reference forms completed (typed or other computerized form) by the physical therapy academic program, a physical therapist clinician and from a resource to verify contributions to the minority community. Application package which consist of seven sets of materials (1 original and six duplicate sets collated) and one original transcript of physical therapy coursework must be received in the Department of Minority Affairs. |
